The only penalty I am aware of is "possible" revocation of one's 
Japanese citizenship. The law is quite clear on this point: the Minister 
of Justice *may* revoke your Japanese citizenship if you are an adult 
(22 or older, actually) and have obtained or failed to relinquish 
foreign citizenship. I have never heard of this actually happening, and 
I don't know how the immigration people deal with obvious cases of 
Japanese adults with dual citizenship (people at the window fumbling 
around with multiple passports, for example). I assume the issue must 
come up rather frequently, in our modern, international times.
