It's understood that this amount is for "pain and suffering." You could
spend a week laid up in the hospital in Japan and not rack up 700,000 yen in
bills.
If the drunken friend wants to be an asshole, he can tell them "fine, show
me a shindansho from the hospital with the doctor's signature, and I'll pay
the bill."
Or he can just suck it up, pay the cash (if he doesn't have it, maybe
they'll accept installments or negotiate), and try not to pass out drunk on
the train any more.
