There are many ways and I'm sure that Brett is not going to give away
all the secrets because that would let spammers figure out ways to
circumvent these controls.

Basically anti-spam ISPs block suspicious mails, depending on content or
source, such as using databases of known spammers or IP addresses, etc.

I think that earthlink.net is one of the best major U.S. national ISPs
that are fighting spam.  Here's more info to get you started.

That is typical of Japanese ISPs.  Go figure.  I would love to meet an
active anti-spam Japanese network admin.
