Kokugikan 03/3623-5111, when it's available. I don't think it always
is, but you could get lucky.

For Kabuki:

Kabukiza in the Ginza.  Call for reservations: 03/5565-6000.  Be sure
to RENT the headphones, if you don't speak Japanese.  It's definitely
worth it.  You'll forget later that it wasn't in English.

National Theatre of Japan, 03/3265-7411.  Sometimes they have Bunraku
puppet theater, which was what I was chasing and found here.  It was
exceptional.  RENT the headphones!
