You should use Bank of Tokyo - Mitsubishi.  They never forget even when 
you want them to.  They sent a very charming OL around in April to 
confirm that I really had closed my account at the Tsukiji Branch of 
Mitsubishi in 1984.  I didn't get to keep the OL, though.  Not even for 
just a little while.  And no tissues, either.  The one they sent out to 
confirm that I was still the same person who had opened an account in 
their Roppongi Branch a bit later in the Bubble but hadn't used it in a 
while I was more than happy to return unmolested.  They gave me a new 
card for the Roppongi account that you have to scan your hand to use and 
then offered me a new Visa and Master Card to go with it.
